“Ultherapy is particularly useful for patients whose jowling is primarily driven by ligamentous laxity and skin looseness rather than significant volume displacement,” says Dr Low Chai Ling. “It tightens the structural layer, which allows the overlying tissue to assume a better-supported position.”
Results develop over two to six months and can last twelve months or more.
Thread lift for jowl repositioning
Thread lifts can mechanically reposition descended jowl tissue upward. Fine absorbable threads are placed along vectors that redirect soft tissue back toward a higher position. This produces a more immediately visible result than Ultherapy in many cases, though the result is temporary as threads absorb.
“Thread lift gives us more immediate mechanical repositioning,” explains Dr Kenneth Lee. “For patients who want to see a visible change sooner, or whose anatomy benefits from specific tissue direction, threads can be a valuable option.”
Fillers for jawline definition
Strategic placement of filler along the jawline can improve definition and create an optical camouflage effect — restoring the visual impression of a defined jaw. Chin augmentation with filler can also improve overall lower face proportions. Fillers do not directly treat jowling but can address associated contour deficiency.
The combination approach
For many patients, the best outcomes come from combining treatments. A protocol might include Ultherapy or threads for lifting and structural tightening, strategic filler for jawline definition, and polynucleotide or skin boosters for overlying skin quality.
“We rarely recommend a single modality for jowl reduction in isolation,” says Dr Low. “The best outcomes come from addressing the structural, volumetric, and skin quality dimensions together.”
Can Jowls Be Fully Corrected Without Surgery?
Honest answer: non-surgical treatments can meaningfully improve the appearance of jowling, but they cannot replicate the results of a surgical facelift in patients with significant soft tissue descent.
“For mild to moderate jowling, non-surgical treatment can produce a visible, meaningful improvement that many patients find very satisfying,” says Dr Low. “For patients with significant ptosis and skin redundancy, surgery remains the gold standard. Recommending the right treatment for the right patient — even if that means a referral for surgery — is what defines good clinical practice.”
